Mandatory Conduct Referral<\/mark><\/a><\/div>\n<\/div>\n\n\n\n

The purpose of the mandatory conduct referral is to provide assistance to faculty\/staff that are experiencing conduct issues that are beyond classroom\/campus management matters. Classroom\/campus management issues are to be addressed with faculty\/staff\u2019s department chair or dean. This form will be reviewed by the Conduct Office.<\/p>\n<\/div>\n\n\n\n

Student Complaint Form<\/a><\/div>\n<\/div>\n\n\n\n

When a student believes that he or she has been treated unfairly by an employee of the College it is our intention to ensure that the student has clearly defined avenues of recourse such that the complaint can be resolved fairly and equitably. It is preferable that the complaint be resolved informally; however, when that is not feasible, this policy will ensure that a formal process for resolution is available.<\/p>\n<\/div>\n\n\n\n

Report a Student of Concern<\/a><\/div>\n<\/div>\n\n\n\n

This form is for faculty, staff, students and the community to report students who exhibit behaviors of concern. Though the term “concern” is subjective, you may feel a student is exhibiting emotional or physical behaviors of concern. This includes non-instructional reports when a student jeopardizes the safety of faculty, staff, or students. This form will be referred to the appropriate student support area.<\/p>\n<\/div>\n<\/div>\n<\/div><\/div><\/div>\n\n

Report Sexual Harassment or VAWA Offense<\/a><\/div>\n<\/div>\n\n\n\n

Report sexual harassment, or other Violence Against Women Act (VAWA) offense such as stalking or dating, domestic, or relationship violence that is so pervasive, persistent, and severe that it would prevent a reasonable person from taking advantage of the educational or employment opportunities at Forsyth Tech. This form is reviewed by the Title IX office.<\/p>\n<\/div>\n\n\n\n

Report Acts of Discrimination<\/a><\/div>\n<\/div>\n\n\n\n

Confidential Resources <\/strong>Schedule an Appointment<\/a><\/div>\n<\/div>\n\n\n\n

Need to talk with someone confidentially? Schedule a meeting with a trained clinician who are compassionate and willing to talk. In-person or teletherapy is available. Confidential Resources cannot reveal the information, except when an applicable law or a court order requires disclosure of such information. Accessibility Resources Request Form<\/a><\/div>\n<\/div>\n\n\n\n

Use this form to request accommodations and speak with an Accessibility Resources Counselor to assist with persons with disabilities or pregnancy-related condition.<\/p>\n<\/div>\n\n\n\n

ADA Concerns, Complaints, or Requests<\/a><\/div>\n<\/div>\n\n\n\n

Utilize this form to submit a complaint or report concern of a Forsyth Tech environment(s) and\/or space(s) that you believe is not accessible or to request an ADA accessible review of your space, event planning, or digital item by the ADA Coordinator.<\/p>\n<\/div>\n\n\n\n

File a Grievance<\/a><\/div>\n<\/div>\n\n\n\n

Complete this report if you would like to file a grievance. The primary goal of this form is to produce a mutually acceptable solution for a grievance through an informal process. When necessary, a Grievance Advisor will move to a formal process.<\/p>\n<\/div>\n<\/div>\n<\/div><\/div><\/div>\n\n

Forsyth Tech takes all reports of hazing seriously. We encourage anyone who has experienced or witnessed hazing to report it. Reports can be made anonymously; however, anonymous reports frequently fail to include sufficient information for action to be taken.
